The regulator, the Care Quality Commission, has said that maternity services at the trust are inadequate and Panorama has calculated that maternal deaths there are almost double the national average

Yet unexplained deaths in children over a year old happen almost once a week in the UK. They sometimes result in parents being falsely accused of harming their child.
